When an OU Health department requires additional space, needs to relocate, changes use of space or seeks to reduce its space within any OU Health occupied space, a Space Request Form must be completed and approved by OU Health's Space Planning Committee.
Once the Space Request Form has been completed and approved by OU Health's Space Planning Committee, UHAT will then process the Construction Request and assign a Project Manager to the work order.
Please reference the Space Request FAQ's below for common questions on Space Request Forms.
Please attach a copy of the approved Space Request Form to the Construction Request being submitted.
What is a Space Request Form, and when should I use it?
A Space Request form is a document that must be completed when a department requires additional space, needs to relocate, changes use of space, or seeks to reduce its space within any OU Health facility. It is essential for managing space efficiently.
Who needs to approve my space request form, and who should use it?
All space requests require approval or sponsor from a vice president level leader at OU Health before they can be submitted through the HUB to Melissa Milburn with Real Estate, Planning Design & Construction Department.
How do I submit my completed space request form?
Go to the HUB. In the search bar, type in “Space Request Form”. Complete the electronic form. Once submitted, the form automatically goes to the Department Director and then to the VP by email for approval. Upon the VP approval, the form goes to Melissa Milburn with Real Estate, Planning Design & Construction.
How long does the space request review process take?
The review process may take up to three months to complete, although it can take longer depending on the complexity of the request and availability of space.
What costs are associated with my space request?
All costs related to the space request, including moving, IT, furniture and painting or any other construction costs are the responsibility of the end-user department making the request.
Will I receive confirmation once my space request has been submitted?
Yes, the HUB will acknowledge receipt of your request via email. At that time, the Real Estate, Planning Design & Construction department will begin the review and set up a meeting to discuss your request.
What happens after I submit my space request?
After submission, your request will be reviewed, and ongoing communication will be maintained throughout the process. Additionally, it will be presented to the Space Planning Committee.
Will I have to present my request to the Space Planning Committee?
Possibly. Departments may be asked to deliver a presentation to the Space Planning Committee as part of the review process.
What should I include in my space request form?
You should include detailed information about your current space situation, the reasons for the request (e.g., need for additional space, relocation or reduction), and any other relevant details that support your case.
What if my space request is denied?
If your space request is denied, you will receive feedback regarding the decision. You may have the opportunity to revise and submit your request or explore alternative solutions.
What do I need to do if I need to remodel or need improvements to my new space?
You should send an email to David Foss, Administrative Director of the Real Estate, Planning Design & Construction department at David.Foss@ouhealth.com
What if I need to change the use of a particular space? For example, changing an exam room to a supply room?
A space request form must be completed when a department needs to change the use to a particular space.
